Xbox 360 is scratching discs, anyway to fix?

Not sure why, but my Xbox 360 seems to be scratching the outer sides of the disc. Like the tips of the circle. It's not that bad since not much data is stored on that part, as you can see the data is closed before the spot it is scratching, but it occasionally scratches the bottom of the disc too. I had just purchased GTA: IV and received it on April 30th, and since the first time I put it in I was receiving the message "The disc cannot be read, please restart the console, or take the disc out and clean it." I do as advised but I get the same message over and over again. The bottom of the disc seems to have scratches as well as the sides. I keep my Xbox horizontal, not vertical, and I also keep my disc in its case at all times, and never move the console with the disc in. I got a replacement from Amazon, and so far the game works fine, but I just took it out and it has those same scratches on the sides. Any suggestions to fix this before this disc is ruined too?
