Writing a "letter of support" for my professor... call him by his first name in it?

I am writing a recommendation letter for my college professor's contract to be renewed. He has all of us students call him by his first name in real life (he's not a doctor), so should I call him "David" in the letter, or "Mr. ___" ? I'm not sure what is acceptable for the committee reading the letter.
