Writing a book, questions? Which is better?

So im writing a book and Im strugglign with the way I want to approach it. The scenes will consist of dramatic scenarios focusing around a boy and his crush who he has become obsessed with. So What is more interesting the book to be written asstraight fiction, so in the world of the novel this actuay is happening. OR these scenes are nothing more than imaginations conjured up by the disturbed boy with the crush?
