insurance had to be covered? For example if someone who required Hundreds of thousands of dollars in treatment was ordered to be covered under an insurance company, multiply this situation times thousands of people. Wouldn't otherwise healthy people under the same insurance company have to foot the bill with higher premiums? The same goes with car insurance, right?
I am not trying to incite any type of anger here I just am somewhat confused about how these new health care insurance rules are going to work.

Yes, in a way that is correct. That is how people game the system, by buying health insurance only when they need it, and then the insurance companies have to pay thousands of dollars for their treatment, resulting in higher premiums. The individual mandate for everyone to have health insurance is supposed to fix this problem, by forcing everyone to pay into the pot their entire lives, rather than just when they get sick. In return, the insurance company will be required to pay for their treatment rather than dropping them when the costs go too high.
