With TMobile how many phones does it take before you get an entirely new phone?

I have insurance on my phone(Samsung Galaxy S), and its still under waranty. I've had 4 of the same phone. and every one ends up freezing constantly, not sending messages, not recieving them or calls, not sending calls. What can I do to get an entirely new phone WITHOUT having to pay the $500 out of pocket for another smart phone
