why is my Dodge Grand Caravan shooting cooling out of the overflow bottle?

About 6 weeks ago was driving my van and it started overheating. After cooled down realized radiator was completely dry but overflow bottle was full and it was not getting pulled out for some reason. Filled radiator and no more problem til about 2 weeks later started overheating again. As long as I remember to add water once or twice a week it is fine. Radiator cap looked bad so we replaced it and it seemed to help but then today noticed steam coming out of the hood and when popped hood steam and antifreeze were bubbling out of the overflow bottle and it looks like came back from the radiator as it was so hot it melted most of the cap on my overflow bottle. But the van was not overheating. I turned it off for few min and it stopped. Next time I drove van it did same thing, so I took it home and parked it and haven't driven it. Any ideas what may be the problem. Radiator was replaced less than a yr ago, along with water pump. Thanks!!!
 
I have a 2001 for escort that is doing a similar thing. I have been driving it about two hours each way to work...every morning and every evening I add about a QT of water...I noticed the other day that the water in the overflow had steam coming off of it. The temp gauge never gets to hot. I've replaced the cap, fixed a leak in the radiator, fixed the overflow leak. There aren't any signs of water on the ground or on the radiator, hoses, etc. I think that I might have a clog in my radiator and am planning to do a radiator flush...hope that works...if not I'm selling it and fast!
