I always wondered that myself! They could add the cream cheese upon request you could use the cream cheese in place of the Mayo, if your wanting to watch fat content. Just have one or the other, not both. cream cheese has only 1 gram less of fat than Mayo. It might be good, but would be messy. But a lot of times messy things are yummy. I often thought if they're going call if Philly cheese, they should have Philly cream cheese in it!
