Why does the pedal keep falling off my bicycle?

When I ride my bike for about 50 meters the left pedal unscrews itself from the crank arm and falls off. I have got the pedal nut as tight as possible.

I can turn the pedal with my hand for a few minutes but it doesn't unscrew itself. It only seems to be when I'm putting weight on it.i.e normal pedalling.

Any suggestions? Thanks
 
The left pedal is a left hand thread. That means you turn it counter clockwise to tighten. Just for the sake of completeness, the right pedal has a right hand thread, turning clockwise to tighten.

If you've got the left/right threaded thing correct, you may need to use a pedal wrench to effectively attach the pedals. This is a place to apply a lot of force to make sure the pedals don't come off. I bought my pedal wrench at Performance for about $10.

You should probably replace the pedal, or the pedal bolt. This seems to occur often to one of my friends and he got a new pedal, and now its fixed. It either your pedal bolt or your crank arm thread, like where you screw the pedal bolt in...Check that for wear and tear.
