Why does my Monitor Image Break Up when viewing motion?

When I view video or play video games on my PC, anytime the monitor displays motion horizontally you can notice the image break up slightly. It's really hard to explain but I can't find a way to fix it. If you need any info please ask. Here's an Example of what I mean: Lets say my monitor is displaying a ball moving horizontally across the screen, on my monitor that ball would break up in half horizontally and one-half would be behind the other trying to catch up. I hope that makes sense.

Specs:

Core 2 Duo 3.0
FSB 1333
GTX 260 GPU
4gigs DDR2
Vista 64bit

Monitor Specs:

3ms Refresh
1920 X 1080p 23in.

could either be the refresh rate of the monitor is too low- try increasing this to the max - windows should advise the max refresh rate from the display settings.

also make sure in device manager that the video card is working ok, try reinstalling its driver.
