Why does my Honda Accord 2002 LX jerks so bad?

When i accelerate to 5-10 mph or 15-20mph and it shifts gear, i got automatic, it jerks bad when i drive fast or something. When i drive fast going down a downhill, and i press brake a little hard, it jerks too. When it doesn't completely stop to around 5-10 mph and i accelerate agian it jerks reallllllly bad. Is something wrong with the transmission? Should i replace it?

Sounds like drive axel is worn out. CV joint need to be replaced. Do you hear a clicking noise when you make a sharp right or left turn? Thats the answer.
