Why does my coolant reservoir cap keeps popping off?

Last week, while I was driving my 1995 Toyota Tercel, the top radiator hose blew up and broke off the part of my radiator. So here's what I did:

1. I replaced my radiator, hoses, and since my heater core is mostly clogged I bypassed it. I bought another heater core, but I am just waiting for it to come in.

2. I drove it around and it seemed pretty good, after about 5 minutes, it started to overheat. So i took out my thermostat and didn't replace it. I also replaced the radiator cap.

3. I drove it around and it didn't overheat till about 30 minutes. I opened my hood and the reservoir cap blew off and coolant was spewing everywhere! i put the cap back on and filled up the radiator again. I drove another 30 minutes and the same thing happened. I also replaced my thermostat thinking for some weird reason the cap was blowing off because i didn't have one.

Im trying to figure out why it keeps blowing off, because im losing all my coolant and my engine begins to overheat! do i need a new one? i never did this until i replaced the radiator and the cap.

why does this keep happening and how can i stop it?
