why does my 2001 honda accord jerk and stall?

it just started happening recently! when i start mt car it is fine....it only happens when i put it in drive or reverse! my car jerks and the RPM's fluctuate and i think the RPM's get so low that the car stalls?

if anybody can tell me what the problem is it would be awesome
