Why does jigsaw kill people and how does he select people to play the game?

i watched SAW movie part1,although i have watched part 5,but the patient who was down in the centre of the ground,what was the link between the operator ,the young guy?
 
if you watch the other 3 movie you will understand so i dont want to ruin it for you cuz its a great series
