Why does it feel like my heart is going to explode when I exercise? Is this normal?

i have been walking for about 5 months now, and still around the 20 minute mark, I always feel like quitting. I sweat A LOT when I exercise (It's literally dripping down my face after 10 minutes.) and I always feel extremely hot. I also always feel like my heart is beating way faster then it should after 20 minutes. I try to do 20 minutes twice a day or 30 minutes all at once, but I always feel like my heart is going to explode and it bothers me. I tried Julian Michael's 30 day shred for beginners today and I had to keep stopping for a minute or two and only made it to the 15 minute mark before I felt that way. I literally have no stamina and even though I've been walking for 5 months, it's not getting any better. When it was warm, I walked outside and I walked with my mom and we did 45 minutes, easy. On the treadmill, I can barely do 20 and have to push myself to 30. Is this normal? I HAVE lost 15 lbs in the 5 months, which I think is terrible for that long. I weigh 153.2 and I'm 5"4'.
I do eat healthy for the most part. My usual is something like..

Breakfast: 1 piece of whole wheat bread with 2 pieces of turkey sausage and an orange. Or just 3 pieces of turkey bacon and an orange, skipping the carbs.
Snack: Non-Fat yogurt.
Lunch: Lean cuisine or turkey sandwich with whole wheat bread.
Snack: Apple, orange, banana, something along those lines.
Dinner: Grillen Chicken, and a vegetable. My mom usually cooks mashed potatoes or macaroni, but I always skip it or replace it with fruit.

I don't drink soda, coffee, etc. Usually just water, water, water.
