Why Do Doctors Not Report Everything in Medical Records??

Ahhh yea dee, that could be a possibility that they got mixed in with someone else's recorRAB. Come to think of it, one time in the PM's office while they were doing their questionairre and putting in the line where they give me sedation...soemtihng was in file b/c they thought I had diabetes and I was like, No i dont, and they were confused. I said id been tested for it many times and never said to anyone that I had it. Then they took some recorRAB to a desk and probably were putting things in the right spot. And that place is like a production line. It's always mobbed in there.
