Why did my fish die? Head all red exploded?

My fish have always been happy and I don't think I have ever had a problem with unhealthy water. The tank is about a month old and I have used water conditioner to treat the water. Today I noticed that one my fish is dead. The water doesn't appear to be unhealthy, its clear, no debris and I don't over my feed my fish. Anyways I noticed that his head is all red and had veins all over. Did his explode? Why?

I agree with gremlin it looks as if the gills have become inflamed because of an irritant, either ammonia or gill flukes which you should be bale to see if you lift the gill cover slightly.
