Why can i not hear people on my xbox 360 headset?

Its got a little cord that i can plug into my controller, and i turn it on, adjust the headsead, and i turn the volume on max... I know i can talk, i just cant hear people (the little speaker icon in the bottom right corner). Also i pressed start during a game and went to the communications section... Everyone should be unmuted but idk if they are or how to change it. In the lobby next to some of the peoples names (to the left of them) there is a N/a symbol (a circle with a slash through it) Does that mean their muted?
Oh and i tried two different controllers...
never was working, was like this since i opened it...
 
i do think that the symbol you are talking about means muted. hit the X and check under your party tab to make sure that you are not under a party chat. if you are, then you will be able to talk and nobody will hear you, and vice versa.
when i have that problem it was cause i connected the headset after booting up. you can try a reset.
