Who's at fault in this fax correspondence?

I have a problem in deciding the person in fault in a matter because I'm biased with some facts.
So, help me out here.

'A' company order greeting card from 'B' company.

'A' faxed an inquiry for the pricing of the cards:
ABC-DEFG 50 pcs $1.00

'B' replied:
It's been a while since you last ordered the cards. The pricing had changed. Please see below.
ABC-EFGH 100pcs $2.20

'A' corrected amount on the quotation and re-fax to 'B':
50pcs (plus a remark at the bottom - We ordered only 50pcs, please correct the amount. Thank you.)

'B' corrected the amount of the greeting card and faxed to 'A':
ABC-EFGH 50pcs $2.20

Then, after that 'A' faxed over the Purchase Order. to 'B':
ABC-DEFG 50 pcs $2.20

'B' replied with a confirmation of receiving the P.O.

--------- a few days later -----

'A' received the greeting card and found out that the design is wrong.
The design is ABC-EFGH instead of ABC-DEFG.

'A' showed 'B' the P.O. and pointed out that he ordered ABC-DEFG.
'B' went back and checked his files and pointed out that 'A' agreed on ABC-EFGH when the amount of greeting card is amended.

So now, who's at fault?
I know that it takes two to tango but I need to know who's should be responsible.
