Where do I include a written parent consent when trying to get a job?

Children under the age of 15 must have a written parent consent.
I was wondering where I include it?
It will probably only be a sentence saying something like "I, blahblah, give my child, blahblah, permission to work" and a signature.
Should it be on a separate piece of paper, or does my parent have to sign a parent consent form of some sort?
