All chants start somewhere, but there's one I don't know where it began...

What city or team started the chant "HERE WE GO (insert team name) HERE WE GO, HERE WE GO (team name) HERE WE GO!!!"

I hear that now everywhere, but I don't know what team or fanbase originated that chant.
