Where can I find a full crop rotation plan for a veg garden - including compost and lime etc.?

I've been trying to plan a vegetable garden. I've found crop rotation plans on the internet but they have'nt included things like when to apply compost.

e.g. potatoes like lots, carrots don't so you time things accordingly.

Where can I find one / can you give me one (a plan)?
