When did authors start writing "historical fiction" and how can we know what is...?

..."fiction" and "non-fiction"?
I'm curious when the term "historical fiction" was created and how do we know if books that were considered "non-fiction" because of its historical accuracy were in fact works of "historical fiction"? Are all non-fiction authors, to some extent, historical fiction writers?
