It's for the PS2, and all I played was a demo a WalMart. You control this girl who can change into many different forms. I remember a giant blue monster that you became to climb a waterfall, a plant-like thing, and something that can curve into a ball and roll everywhere. You could also press L1 or R1 for the character's wings to appear and she hovers about.
It's been years since I played it, but now I really want to try to find it. And if you don't know, then don't even bother answering.
