what's the name of the test that pyschologist and psychiatrists give to test

for mental disorders? i took it a while back. it was about 600 questions and it took like 5 hours to finish.. there were so many questions because it was to check for consistency.
it scored me to have sever depression when i got the results

it's something like the d-12 test or something.. can someone help me out with the proper name?
