What's the best approach to getting a smooth paint finish?

I've just had the door stops replaced on all my doors due to fire regs.
The new doors stops are thinner than the old leaving areas of bare wood next to areas of heavily painted.

What's the best approach to getting a smooth finish? Do i need to sand it all off? should I fill it? Over 70 years of paint in most areas!
