What's going on with my betta fish?

Well, my poor little betta has been through alot, partly my fault and I am mad at myself for it, anyway. He has hurt his fins 3 times since I first got him, 2 times were from a filter in my tank, the first time from the filter I fixed it and put sponges in it (and treated him with aquarium salt and water changes), the 3rd time I have no clue why, but that was not too long ago and since then I started using a 100 percent natural medication for freshwater fish (and bettas) that treats fins and all kinds of problems. I ended up putting hose over the filter valve so there was no way he could be harmed and his tank cycle would not crash (I thought about taking the filter out, actually I did, but put it back soon after, so I hope that didn't cause any problems). I did a very large water change after I stopped using the medicine (around 10 days), and then the weird thing is he got a white spot on each eye and before that while I was medicating him, he got white spots on the ends of his fins where they were torn. Now, he still has the white spots on his eyes (I'm not using the medication anymore), but the white spots are gone off his fins and they show signs of healing.
He is acting happy and excited and eating..
Today I am keeping his tank very clean for now while using Prime conditioner, I did a small water change, and his tank smells very fishy. WHY?
It is a cycled tank and i've done frequent water changes, so that shouldn't be a problem.
My betta, when I added fresh water, did unusually stay underneath the water while I was pouring it in, he acted like it felt good on him.
I'm confused, the tank is cycled with live plants and everything, I could notice the smell in the tank on the other side of the room.
Should I just put him in a quaranine home with daily 100 percent water changes till I figure out if there is a problem with his home? I know 100 percent water changes in a smaller home daily will help him tremendously, I know he will always have clean water. And not to stress him, i'd be doing 50 in the morning, and 50 at night and probably add a plant in for him to hide in.
I broke my last test tube so I have to get more, for now I am doing frequent water changes for that and to help him, and I am hoping to get the test tubes tomorrow. I think I will. But that's why I mentioned putting him in a quarantine home.
He is in a 5 gallon tank and he has no tank mates.
It is heated to and I have a thermometer in there, the temperature stays at 76-78 degrees.
