What would cause an 02 chevy 1500 to rev really high when it is cold?

When the over night temp is below 40 and I start my truck in the morning the engine will race like I have my foot on the floor, then it will slow down and almost die. Any ideas? Could it be the O2 sensor?
 
Computer system trying to compensate for the extremely cold weather.

If you have a block heater on the truck, its possible that the system is sensing a slug of warm coolant and the system is is thinking the engine is warmed up and can cut back on fuel delivery, causing a lean condition that is not compatible with the extremely cold air entering the cylinders.
 
The answer depends on whether your motor has a cable operated throttle or electronic throttle.

The cable operated throttle uses an Idle air control valve IACV to bleed air into the engine to start.
An electronic(throttle/fly by wire) system uses the electronic throttle body as an air bleed.

The controlled air bleed in controlled by the computer to adjust the idle.

It sounds like either your throttle body or IACV is experiencing what we call "Icing".
Basically the metal gets cold enough (or actually freezes) that the valve or throttle blade will stick when it's first turned over.

sounds like it is sticking when first started and then when it unsticks it doesn't stop the valve at it's adjustment point so it will idle correctly.

Basically,you need either a new IACV or a new Throttle body.

I don't remember the exact year GM started using throttle by wire and not all of them had it.

A stop at almost any major chain parts store like Autozone will tell you which one you have and the person there more than likely can point it out to you.

Unfortunately there is no real diagnosis for icing-as once you start it up it will usually be fine afterwards until it has another long cold soak.
So you'll need to simply replace the faulty part before you know it's the issue.
 
could be the air temperature sensor which give a false reading to the ecu so you have too rich a mixture of fuel and air
