What would cause a car to lose control and/or lock it's gas pedal?

Ok so I am only 16 and I just witnessed an accident right in front of me within feet away from me. Third time within three days. This one guy, an elderly person, was backing up trying to straiten his car out while he was trying to park right next to us, he was backing up slowly than out of nowhere he goes full speed backwards crashes into a building, his airbags pop open with his hands up away from the steering wheel than the car drove foreword at 30-40 miles a hour and hit a tree. My mom and I got out of our car to see if this guy was ok, within feet from us. He turned out to be ok. We was bleeding a little, mostly in the ear and arm, but he was ok he said, he was walking around, we called 911 just to make sure he was ok because he was bleeding. Anyway he said the gas peddle got stuck and he lost control of the car. He said it went forwards without his command. It was a 2011 Honda, It was also a rental car. He ruined the car pretty bad. How would a cars go foreword without the drivers command, and how would the gas peddle get locked like that? My mom said the car looked as if it had a mind of its own. I'm still kind of confused on what just happened. It was very odd to see a car act like that, even though his hands where away from everything.
But he still said the gas pedal got stuck.
 
He confused the brake pedal with the gas pedal. Its a problem with older people, they also forget even how to start the same car they have been driving for years. There are plenty of real accelerator stuck to the floor incidents. I once had it happen to me in a 1957 Ford, the secondary butterflies in the carburetor were connected by a easily bent rod. It locked the secondaries full open. So I thought, OK, all I have to do is turn off the ignition. Good in theory, except the engine was coked up with carbon, and it no longer needed a spark to keep firing ! I was finally able to bring it to a stop with hard braking, but it was a scary experience.
 
A car cannot do that on its own.

When a car appears to do that, it is usually because the driver tried to hit the brake, missed, hit the gas instead, thinking it was the brake, and thought that the gas was stuck, because it was down when the driver was not trying to press it.
 
only thing i have seen myself is when i was driving car companies make sh**y cup holders my water bottle fell over left when i was making a right turn and then got stuck under the brake petal therefore no breaks lucky no one around and wasnt going that fast to begin with and was able to reach under because the road was striaght. it could have been worse,,,,,,o-o
