What is Verizon's return address?

I ordered something online from Verizon and need to return it. The return address on the box is incorrect and when I called Verizon they first said it was indeed correct and then double checked and gave me a different address. UPS said both addresses are incorrect. I don't have the return label and would prefer not to have to go to a Verizon store. Anyone know the address?

One on box: 4320 North Sylvania Avenue Fort Worth, TX 76000

Lady on phone said: 4801 Mercantile Dr Forth Worth, TX 76000

UPS said something about the zip being wrong.
 
The zipcode on my return label is 76137-3635
The actual label says:
Fort Worth
4801 Mercantile Dr
Fort Worth, TX 76137-3635

Yes, look at the address, but dont go there. No we can never go back to the old forums. The old forums have changed. We dont know whats happenning there. You would only endanger us all by going there. I can only imagine what waits for us. OH THE HORRORS, THE HORRORS
