What is the perfect posture/position for a push-up?

I need to do as many push-ups as possible in one go. However the posture/position needs to qualify as "standard".

How far apart should my hands be?
How far apart should my feet be?
At what speed should I do them?
How should I position my head?
Should I tense or relax?

Any other tips would be greatly appreciated...
