What is required to port a number from Vonage to t-mobile?

I want a port from Vonage to t-mobile to go smoothly.

lists the following info is required:
Vonage account number,
10-digit phone number
Zip code
account pass code, if applicable.

Here's my 2 questions:

Is the account pass code the 4 digit pin I use when I call in or the password I use online to access the account?

Is the Vonage Service Provider Address required, and if so, does anyone know what it is for Vonage?
