What is a income verification form?

One of my co-worker need our employer to sign a income verification form, but our employer refused to sign it. My co-worker said that she needed the form to be sign to receive financial aids for her child. I'm just wondering what is a income verification form? And is there any reason why a employer wouldn't want to sign one?
