What is a correct/polite way to write an e-mail to a teacher? I am in high school.?

I am jumping from spanish 3, which it took last year?, to spanish 5, skipping spanish 4. in order to do it, i have to do homework and power point, and later take a test on summer work. I just a got a mail from my teacher that said that the test will be in august, with 2 make-up day and she asked us to reply by tomorrow. I will not be able to come at any of the available days. How do I write it in a polite way? how should i start the letter (Dear... or whatever, English is only my 3rd language and these details are still hard for me)? any help would be appreciated
 
Dear (Teacher),
Unfortunatly, i will not be able to attend the test date or any of the make-up dates because(Whatever Reason). I am still very interested in completing this course over the summer holidays, so I am willing to take the test on any other date that fits your schedule, besides the days that I'm gone. I am gone between (date) and (date). Thank you very much.

If the teacher e-mailed you I would e-mail her back with your name in the subject and something like sorry i can't make the test dates and then say Ms. (Mrs.) ... I received your e-mail with the test dates and I'm unable to make any of the dates because ... suggest a few dates around that time during the week and say if they don't work which day would work for her. Thank her for her understanding and put your name at the bottom.
 
Don't worry. I go 2 a tech school where we have 2 write emails to our teachers all teh times.
