radiologist did not like what they seen on a breast ultra sound and requested an mri right away. mri department stated on day of proceedure that they had not gotten pre authorization but were going to go ahead anyway. insurance has denied the claim as not medically necessary. so am i now responsible to pay? should they have had pre authorization prior to scheduling? who do i contact to find out?
