My 2nd saddest goes to the final scenes in Mahoromatic, but they tell you it is going to happen from episode one.
The all-time sad for me is "Lucy's Confession," from Elfen Lied. While EF has a pretty good EngDub, I thought I'd post the more powerful Japanese dub:

The odd thing about Mahoro Andou is that they TELL you that Mahoro is going to deactivate in Ep. 1 and they give a countdown to deactivation at the end of every episode for the two seasons. Still, I hated them killing her off, and I hated the manga more for leaving the fate of Mahoro ambiguous. So, this is the first time in an anime that saying the main character is going to die does not count as a spoiler.
...and, at least in the anime, Lucy's "death" many not be a death at all. Remember, who is the girl at the gate when the clock starts again?
