What could be wrong with the heater in my car?

My car has four settings for heating and cooling. (1 is the lowest, 4 the highest.) Several months ago, I was able to turn my heat on a low setting by turning it from "off" to "1". Then "1" stopped working and I had to turn it to "2" just to come on. Since then "2" and "3" have both stopped working, so now I can only either blast it on "4" or nothing at all.

Any ideas what may be wrong? And more importantly, how much will it cost me to repair?
