What circumstances deem it necessary for a newborn to be taken into state custody?

I know that if the mother at birth or anytime during pregnancy is tested positive for drugs her child will be taken at birth. What are other things that the state will take your newborn for?
Edit: I'm talking about in general, why babies are taken away from their mothers.
I'm also wondering if they take anything in your past personal life into consideration when deciding whether or not to allow you to have your child. I don't have the cleanest criminal background history, but I have nothing on my record of any child abuse or anything like that. Just things like wrecks, stealing, and drugs (and I don't do drugs at all anymore or steal). And if they take age into consideration and if you're single. I'm a single teenager so I'm afraid they won't allow me to have my baby.
