what are interesting happenings for my book?

I am writing a book called 'Smile Summer'. Its about an orphan who gets put in a foster home of a wealthy family with a famous dancer as a son. But he doesn't actually like his life and she conviences him to runaway with her. Soon enough the boy's parents have police chasing them and they end up falling in love. But...I need a big happening for my book. Like a problem , like all books have. Ideas please? :) thanks.
 
1) The boy has some kind of health issue where he needs meds for say, diabetes, epilepsy, etc. Whatever! Some kind of health problem that needs daily attention and he didn't take his meds with him. They can't get the med without a prescription....which leads to #2.

2) They hook up with some other kid (runaway) who promises to help them,but don't realize he is a trouble maker. Soon enough they find themselves trying to elude this kid and the law also.

3) The mom makes a plea on TV and asks them to come home because the father is dying. It is a ploy to entrap them and take them back home. The boy is debating whether he should go back or not. The girl doesn't want him to go back because she feels it is not true and if he goes back they'll never see each other again. They start arguing.
 
They go to a restaurant together to get something to eat (make it McDonalds or something) and someone recognizes them and calls the police, so they have to make a run for it.
Later on, they could somehow get separated and end up in different towns? Just an idea.
