what am i doing wrong with my swing?

i have been playing 15 yrs now and always had a bit of a hook.but the last year or so on my tee shots they get 4 ft off the ground and nose dive after 90 or 100 yds...they go perfectly straight now and thought i had corrected looking up during contact

You are topping the ball this is common if your clubs are to short. try looking at the back of the ball instead of the top. if this don't help get a real lesson
