Weird stuff coming out from 'down below'?

Ok this is a bit embarressing.....but I really want to know what it is.

When ever I go toilet, I notice some weird fluid in my nickers, its white or yellow or clear....usually clear....it's thick and horrible and it happens every day, I am always having to change my underwear, what is it??? How can I stop it??
