Verizon LG Dare insurance PLease help!!!?

I currently have the LG dare and its under insurance, it has many issues right now

if i brought it in under insurance i want to know will i get a new LG dare or a totally new phone,

will i have a choice in the new phone?
Has the lg dare been discontinued?
if i dont have a choice in the new phone , what phone will i get?

btw: my contract
