T-Mobile: Nokia C3-00 Puk Code? Customer Service?

I got locked form my phone, and now its askin for a puk code. I heard i am suppose to call customer service. But how do i call T-mobile? Anyone have their number? ALSO when i do contact the customer service, what information do i need to provide? (example my address, account #....ect)
