Switching from Active to Reserves in Army?

I enlisted in the army last month and swore in for a four year active duty. I'm in the delayed enlistment and I don't leave until January.
Some personal things have come up, and thinking of leaving literally makes me sick. I've been avoiding my recruiter and I've stopped working out (I am underweight and have not met my weight and it's looking like I won't).
I know I must honor my contract, but right now it's not appearing realistic. If there was a way I could switch to reserves, then I would be very happy. But if not, I might have to discharge myself.
How would this work? If I discharge myself before I leave for BT, would it be dishonorable? Or would it be considered an absense or going AWHOL? Is it too late to switch to reserves? This is making me very depressed.

I have a couple things going on right now. I'm currently trying to recover from a serious injury I enlisted into the army with. I passed my physical, still early in the healing process (I was kicked in the femur by a horse. Nothing broken, but there was a very large laceration and muscle damage.), But despite all my efforts to strengthen the muscles, they seem to be deteriorating. Spots around the area have become knee, the damage done to the knee (hyper extension) is not reversing but getting worse, and there is a very visible dent in the muscle that was not there before. I have also become depressed and would like to seek professional help but would like to wait to see what will happen.
I was rushed into swearing in, and did not realize it was exactly what it was. They lied to me and just told me it was the first step, not the official swearing in. (I only went to MEPS once, to do the ASVAB, physical, and swearing in.)
