Strange bumps under middle toenails

I've noticed for a long time now that whenever I cut my middle toenails, I can't cut too close to my actual toe or else I get a very sharp pain, as if I am touching something irritated under my toenail.

Curious as to why this was only happening on my middle toes, I recently took a look to see if there was anything out of the ordinary.

What I see almost looks like an in-grown nail of some sort. Under both of my middle toenails are a small bump with a hard, whitish surface similar to the material of a toenail. If I touch it with anything, it sends a sharp pain through my foot.

Does anyone else experience this, or know what it is? The most bizarre thing about it is that both of my middle toes, and only my middle toes, have this strange bump underneath them. They make cutting my toenails a longer, more tedious experience, since I have to carefully make sure I don't cut those nails too short, or else it will hurt a lot.

I appreciate any advice you can give me.
