Simple House Rules / Consequences & Rewards?

Too long a story to make short, the easiest way I can say this is that I need to come up with five simple rules & their consequences, as well as rewards, for a teen in an emotionally fragile situation.
(These are for a 12 year old that will be re-entering the house after having been gone for 2 weeks, learning a better behavioral system.)
They need to be simple and concise. Here's what I have so far:

1) (too difficult for her to maintain a whole wardrobe of clothes)
"You will have one week's worth of clothes in your closet. You are responsible for keeping these where they belong (clean in closet, dirty in dirty clothes area). As long as you are following these rules, you may replace 2 outfits at a time, that will remain put in a different area."
Can you think of a consequence if she has to keep getting redirected to put them where they
belong? I will keep a box of more relaxed, after-school clothes. How about, "After-school clothes will only be offered when clothes are properly maintained, (none on floor,etc.)"

2) (b/c she had become explosive to people simply calling her)
"Respond respectfully when called (answer kindly, no yelling.)"
-- Consequence for failing this? (time out to give her time to answer calmly?)

3) (b/c her teeth look like wood but smell like trash)
"Teeth are to be brushed both morning and night."

I can't come up with other ones. I was thinking of a points reward system for regular, maintenance type things, but I'm drawing a blank.
I really could use some help...............please?
