Should I tell my friend that wears her clothes way too small?

My friend whose birthday is coming up tends to wear clothing too small and inappropriately. Its embarrassing and we get stares because of her appearance. She wants to go out bar hopping but I've had it with the clothing and the bad makeup, and did I say its really embarrassing. Should I say anything and how should I say without hurting her feelings?
