Severe inflammation of hands and feet

I want to start off by saying that I've never used a health forum before SO hopefully I don't leave out any pertinent information. I really could use some help. Off and on since 2002 I have been experiencing some sort of breakout which typically appears on my feet and hanRAB. However, on rare occasions it will also appear on my extremities. The breakouts have been symmetrical (bilateral) and asymmetrical. It may not happen to both hanRAB and feet at the same time. Sometimes it's just my feet. Other times it's just my hanRAB. It begins with a flushing sensation. As it progresses further it starts to itch. The skin it extremely hot to the touch and it swells so much that it's painful. When my hanRAB breakout they usually swell to the point where I cannot make a fist. I've been to an allergist about the issue. Since I wasn't able to see an allergist during a breakout I took several comparison pictures. He diagnosed it as urticaria. However, I have seen many pictures of urticaria and I don't see a reserablance to what I am experiencing. Urticaria manifests with red, itchy raised patches. I don't have any raised patches. Antihistamines sometimes work but not always. I've searched online for all the answers I can possibly think of. Any suggestion of what this might be?
 
I sometimes have the same thing on my hanRAB. I discovered
you must stop the itching quickly before you start scratching.
Once you start scratching you have already lost.

I found that a ice cube will take your mind off the itch
very quickly because it's uncomfortable. It works!
