Save my fish! Betta Fish help! Fins falling off!?

I got a Betta fish 2-3 weeks ago, and recently I noticed that it seems like his fins (the long ones as well) are almost disintegrating, just falling off. I have been following proper betta care procedure as far as I know. I have a small tank (Marina brand) with gravel at the bottom and a fake plant and a small removable heater- I keep the temp between 75-80F. Do partial water changes every day and/or full water changes a few times a week using bottled water and adding the Nutrafin Betta Plus water conditioner. Also noticed his gills have become more red underneath and scaly on top. I feed him 1-2x a day with a few flakes- as many as he can eat in a minute or two. This happened with a previous Betta fish of mine (same setup, etc.) right before he died. Can anyone help save my fish?
 
You betta has fin rot. The best treatment id Jungle fungus clear. Thea's tablets treat 10 gallons but can be broken into smaller proportions.

You should only feed you betta once a day inVeryy small proportions (their stomach is only the size of their eye balls) because from over feeding they can getswim bladderrdiseasee.

If it has ick it will have white tiny dots all over it as if someone salted you fish. What it sounds like it may have fin rot and gill flukes since his gill are inflamed. As far as feeding is concerned don't use the flakes get some betta pellets soak them in a table spoon of aquarium water that you have removed and put them in a cup before feeding, also you may just want to use that cup just for him.

Now for the treatment get some aquarium salt 1/2 teaspoon per gallon of water with some ParaGuard for a week

1) If it's not cleared up take a break from the ParaGuard 2) and continue with 1/4 teaspoon of salt for an extra week, 3) and after that return to the ParaGuard for another treatment
 
stop doing the full water changes a few times a week. you only need to do a 10% water change per week. Doing these water changes is adding stress to your fish, hence the reason his fins are falling off.
