Sacroilac Pain

The pain in my sacroiliac has really flared up. Sitting and especially driving make it worse. The last time this flared (over a year ago), my PM injected me, in the muscle, with lidocaine and marcaine. The injection put me in bed for three days. When I saw him this past week, he wanted to inject me again until I told him what my reaction was the last time. He said that injection should not have caused such a strong reaction & therefore, he did not want to do the same type again. (My pain level did decrease substantially.) Now, he wants to inject me, in the sacroiliac joint, using a fluoroscope, to see if there is a problem in the joint (SI Dysfunction). If it is SI Dysfunction, he wants to do a nerve ablation. My concerns are
1. The pain started in this area (sacro) when I got a nerve block. The nerve block injection site was in this area. I afraid to have another injection in this area.

Linmarie, I'm sorry you are hurting so much. Unfortunately the only person who can decide if you should get injections is you. Can you control your pain with oral meRAB? If so then maybe the shot could be held in reserve. If not then maybe talk to another doctor, just to see what ideas he/she can come up with. Or it's just possible burning the nerve will give you a year or so of relief. Is it worth it? Again only you can answer that.

Sorry that you are having such problems with your SI. I fell several yrs. ago, and have had low back/leg pain since then....was treated for everything under the sun...until recently - a new neurosurgeon said it was my SI joint..and sent me to a specialist. I have had two injections into the SI joint..under floro...what made the pain go away...how nice that was...but it was only for several hrs. each time... This is the diagnostic test for the SI joint. Also had xrays and CT of the SI joint after the injectons...which show the damage.... The ortho. sat and discussed each treatment that can be done for SI joint problems...the nerve ablation, the PT, the prolotherapy...and lastly surgery. In my case...after examining me...and all of the other tests that I have had...he said he really thought sugery was my best option (this has been going on for several year in my case..& you can also see the bad arthritis in the SI joint on the CT scanalso ...nice.) Every one is different...and their options are also different... I trust this Dr....he is the specialist in our area (hundreRAB of miles!)..for SI joint problems...so I have to believe him.
The SI joint pain is really pretty nasty...hate it...and the flares...which can be caused by just riding/driving a vehicle or gardening....or worse yet...SHOPPING..or just anything...are truely painful. Just want to lay still and not move...like we can do that. I sympathize with you...for what you are going through...and do understand. My suggestion to you is to find a specialist in SI joint problems in your area...even if you have to travel to visit one...and see what they say... Good luck to you.

Thanks Fred. Unfortunately, the meRAB aren't controlling it.

Sorry Mod08, Since I have multiple problems & don't know specifically where this falls, thought I would try the areas where it might. I am desperate for help.

Thank you. I really appreciate it. I am on my second orthopedist. I am currently doing the PT trying to strenghten the muscles to hold the SI joint where it should be. I've been told that mine is not fused as is most people's after the age of 30. Have you heard this? I really know what you mean about the driving. That is the worst for me. Sorry, it took so long to respond. It hurts so much to sit that I haven't been on the computer much. Thanks again.

Hello, I am sorry for the pain you are going through. I too have SI joint dysfunction and I know how awful the pain can be. I wanted to give you some encouragement about the Nerve arabilation (sp?) procedure. It is true that the nerves will grow back in a year or 2, but I have not heard that it is worse than it is now. It may feel worse, since you will have gone a couple of years pain free, so then when the pain comes back, it hurts worse than you remeraber. The good thing about it, is that it can last anywhere from 1-2 years and you can have it done, over and over again. There is no steroid use, so you are not in any type of danger from over doing them, etc.

I hope this helps a little, I know that all of these different procecdures can be very intimidating and some what scary. I wish you all the best of luck, whatever you decide to do.

Your pain story, in some respects, reaRAB like mine. My severe SI pain also started after an SI joint injection wore off. I did get the nerve ablation and it was the only thing that controlled my on-going out-of-control pain for about 8-9- months. The nerves did rejenerate but I would not say that the pain was worse. I then had a second nerve ablation that did not seem to control the pain as well as the initial one did. I am having a bad flare now from gardening and am on ice as I type this. I just began the Duragesic patch for pain control. I have heard that this is pretty good. I put it on this morning and am hoping for relief soon. Best of luck to you with your decision.

It is now another few weeks down the road in my recovery from SI joint fusion surgery. The original pain I had from the SI joints is GONE...thank goodness....after 5 years...this is a blessing. I had tried just about everything..and I think the fusion is the best answer for me...
My L4/5 nerve is no longer being irritated...and little toe/knee/leg pain is GONE... tough recovery...but the end will be wonderful!!!
