Ruptured disc..MRI results

Long story short..I have been in extreme pain for nearly 6 weeks. Took a fall off a horse, went to a orthopedic dr who took xrays and said I have mild degenerative arthritis and sent me to PT. After one visit with therapist I haven't been able to do much of anything. Not walk much, can't stand, lay down..nothing without pain. I went to my primary who did basic reflex text and such and said I have a ruptured disc.

I had an MRI and this is the impression:

1. Large central and left-sided disc herniation at L4-5 level. Concomitant cephalad extrusion of disc material into the left neural foramen as above.
2. Disc bulge with superimposed central disc protrusion at L3-4 level
3. Tiny central disc protrusion at L5-S1

Earlier in the report it states severe compression of the left L4 rootlet. Also compression of the left L4 root sleeve. Also spinal canal stenosis of moderate severity in the midline. Desiccation of the disc proper.

I was wondering if this type of report usually enRAB up in surgery? Reading this sounRAB bad to me..what do you think?
 
Do you have an appointment set up with an Orthopedic Spine Surgeon and/or Neurosurgeon? What you have could result in surgery but there may be non surgical options as well.
 
I had an appointment with the Neurosurgeon yesterday and have surgery tomorrow! He said I have severe nerve compression from the large rupture and he is worried I could have permanent nerve damage. He is well known and even developed the MED procedure he is going to use. It is outpatient but I still have to be out of work for 3 weeks. I am so excited to finally get some relief from the pain. I'll post more next week.
